Medicaid in NH

In New Hampshire, the cost of bringing care into your home, care within a licensed Assisted Living Program or care within a Nursing Home Facility may be covered by Medicaid. The Home and Community-Based Care program, also known as Choices for Independence (HCBC-CFI) is the Medicaid Program that offers a variety of home and community-based services. This program also includes care within a licensed Assisted Living Program.
The 2019 financial eligibility guidelines are a gross monthly income of $2313.00 or below. The individual’s income, after expenses and deductions, is used to offset the cost of care, with the balance paid by Medicaid.  The asset limitation is $2500.00, per applicant. In addition to financial eligibility, applicants must meet medical eligibility.
Opening New Doors Senior Living Placement Services is able to help families find In-Home Care/Home Health Care agencies and Assisted Living Programs that accept the Choices for Independence benefit. We do not advise or assist with completing the Medicaid Forms. We can direct you to state agencies that provide free assistance with completing the paperwork or Eldercare Attorneys who are able to efficiently file the paperwork on your behalf.
